If you are interested in occupational therapy for your child or if you are referring a child, please fill out the below form and our team will respond as soon as possible.
 


If you are not the parent or caregiver of the child, please ensure you have gained permission from parent or legal caregiver prior to submitting the below form.

Please note we are not a crisis service, if you are experiencing an emergency please dial 000. If you require crisis support please contact the relevant service, these numbers can be found on the NSW community and justice website under "general support and counselling numbers."

Are there any court orders or parenting arrangements we need to be aware of?
Does your child have any allergies?
Are there any concerns with having to wait in a busy waiting room?
Does your child have any difficulty finishing or leaving appointments?
Would you be interested in Telehealth or OT Assistant sessions?
Does your child present with unexpected aggressive or violent behaviours toward others or themselves?
Is your child currently seeing another OT?
Does your child have any sensitivities or dislikes we need to be aware of?
Does your child ever abscond (run away) or wander?
Are you a family who has received/are currently receiving services from Playsense OT ? (You may be a returning client or already have another child in therapy)
